As a dues paying member you can purchase a permanent PebbleCreek Singles Name Badge either by filling out a form at a monthly meeting -or- by ordering one on-line.  Plastic name tags can be obtained for free at the monthly meetings
 
To order the permanent tag on-line you must do two (2) things:
 
  1. Complete the information below, and
  2. Send a check in the amount of $5.75 to the PebbleCreek Singles Treasurer:

Upon receipt of the $5.75 and verification that your membership dues are current, your badge will be ordered.
